YDP is an app to access Careplan and other features from your mobile. You need to have a working account with Your Doctor Program, LP and the QR card issued by YDP. Scan the QR card with in-built camera and you can access your Careplan. This app comes handy if your would like to share your Careplan with any healthcare providers when you are in their office.
